- Form from www.smszu.edu.pk or the Registrar's Office; non-refundable fee Rs 3,000 (BPS-17-19), Rs 2,000 (BPS-8-16), Rs 1,000 (BPS-2-7) at UBL Khuzdar or by draft to the Registrar.
